Targeting tumors

“We are trying to develop a robot that is able to take a biopsy in the liver, while the patient is in the MRI scanner.

Currently, the patient goes into the scanner, an image of the patient is made, and doctors decide where they want to take a biopsy. The patient is then removed from the scanner, and doctors use a needle to get a biopsy. The patient goes inside the scanner again, and they check the image to see if the needle is placed correctly. If not, the patient is removed again, and so on, until the needle is in the right place.


What we want to do is to make a robot that is able to place a needle in the patient and navigate it to the right location in the liver. For biopsies, one uses thin needles. Whenever you insert that into tissue, the needle starts bending. What we want to do is to add an active element on it, so that one can adjust the direction of the tip to steer it to the right location. This research involves cooperation between several parties, including companies and universities. I’m mainly looking at the technical development.

So far, we’ve focused on shape sensing, and we want to go further with force sensing. Force measurement can be beneficial. When you experience forces around the tip, you know that the environment – the patient – is moving. So, in that way, you can tell whether the needle is still in the right place, and you could compensate for that. We want to come up with a good steerable needle, and we need to come up with a prototype that really works and assess it.


If this development is implemented, I can imagine there will be some kind of interface for the clinician, on which he can see the MRI images, as well as the shape information, and maybe forces at the tip. He could navigate the needle by joysticks.

At the same time, the patient is in the scanner, the robot is placed over the patient’s legs, and a needle is steered into the patient’s body. This would allow for all kinds of interventions, like taking a biopsy, but also perhaps even inserting drugs, heating up tissue, or whatever else the clinicians want to do.

One could even imagine that while the patient is in the scanner, doctors can diagnose a certain kind of disease, immediately decide what treatment is best, and then execute it. That would really be fast and efficient! But that’s still for the future.”

Van Bruggen volgde aan het begin van haar derde studiejaar een minor bij Industrieel Ontwerpen. Nadat ze daar de nodige vakken had bijgewoond, wilde ze als eindproject ‘iets’ doen met ontwikkelingslanden. Via haar begeleider kwam ze in contact met een oud-student die tegenwoordig bij Artsen zonder Grenzen werkt. Hij introduceerde haar bij Mzungu, die met allemaal identieke speelgoedauto’s in Kenia op de markt staat. “Die auto’s zijn gemaakt van gerecycled plastic en lijken zo op elkaar dat er wel een fabriek achter moest zitten”, vertelt Van Bruggen. “Dat bleek inderdaad het geval. Het plastic wordt verzameld door daklozen. In de fabriek werken negen mensen die het op kleur sorteren en het verder verwerken. Het plastic wordt versnipperd met een machine. Daarna wordt het gesmolten en in mallen gegoten.” 

Dit bedrijf fascineerde Van Bruggen zo dat ze min of meer per toeval de richting van het plastic opging. Ze startte een mailwisseling met Mzungu en beloofde hem te helpen met kennis en het zoeken van sponsors. “Hij is dolblij en ziet mij als een engel”, vertelt ze lachend. “Zelf vind ik het een goede zaak om plastic te hergebruiken. In de oceaan drijft nu al plastic soep.”

Speciaal voor haar maakte Mzungu een bedrijfsfilm waarop precies te zien is hoe de auto’s gefabriceerd worden. Met deze film ging Van Bruggen langs het bedrijf Hermion, dat sorteermachines voor plastic verkoopt en langs Naber Plastics, een kunststof spuitgietbedrijf. Ze bekeken de film en gaven nuttige tips en adviezen. “Zo adviseerde Hermion om de mallen eerst in te vetten. Mzungu nam deze tip meteen ter harte, en het blijkt inderdaad een verbetering te zijn.” Via dit bedrijf kwam Van Bruggen ook in contact met het plastic recyclebedrijf Axion in Manchester. Daar doet ze tussen 13 en 29 april haar bacheloronderzoek naar de degradatie van plastic. 

Op allerlei manieren probeert Van Bruggen haar inzicht in plastic te vergroten. Ze volgde al het vak chemische nanotechnologie bij scheikunde. Voor de komende periode staat het vak designing and manufacturing of plastic products op het programma. Dit alles met het doel om het bedrijf van Mzungu optimaal te kunnen helpen en uiteindelijk zelf een product in de markt te zetten.

“Ik wil graag leren ondernemen. Wellicht zet ik een product in de westerse markt. Ik heb nog geen concrete plannen. De speelgoedauto’s kunnen niet zomaar op onze markt geïntroduceerd worden, omdat de grondstoffen in het plastic getraceerd moeten kunnen worden. Dat is lastig bij gerecycled afvalplastic dat is ingezameld door daklozen. Toch wil je het wel op deze manier blijven doen, zodat de daklozen hun werk niet verliezen.” Ook zou het kunnen dat Van Bruggen een product ontwikkelt voor de Keniaanse markt. Ze denkt bijvoorbeeld aan een waterpomp van plastic. 

Voor het ontwerpen van het uiteindelijke product zocht ze contact met ontwerpbureau Ideal & Co, dat wel vaker projecten doet in ontwikkelingslanden. Dit bureau beloofde haar te helpen. Voordat het zover is, is er echter nog het nodige werk aan de winkel. Van Bruggen gaat nu op zoek naar sponsors voor Mzungu’s bedrijf. Voor zichzelf hoopt ze een beurs te kunnen bemachtigen. Na de zomer volgt ze eerst het vak ‘bedrijfsanalyses in ondernemerschap’. Dan kan de onderneming beginnen en reist ze voor twee maanden naar Kenia, om zelf te zien hoe het er in het recyclebedrijf van Mzungu aan toe gaat en om de Keniaanse markt te verkennen. Inmiddels is ze helemaal into het plastic: “Het komt op mijn pad en ik moet er wat mee.”
